The personal future action plan is designed to facilitate the young person’s ability to achieve their full potential. This will empower the young person to engage positively in all learning and life experiences, ultimately achieving their short term and long term aspirational goals. It incorporates the values of school, home and the community with the young person’s specific individual needs. The success of this process is the responsibility of the stakeholders who are all obliged to collaboratively contribute to the design and implementation of this plan.
The implementation of the plan is facilitated through learning experiences, assessment, adjustments and resourcing throughout the student’s learnings at Townsville Community Learning Centre.
Personal future action plans will be reviewed by the student’s case manager (who is generally the class teacher), care givers and other relevant stakeholders.
